Not medical advice
Content on this site is for general information only and is not medical advice, diagnosis, or treatment. Using this site, submitting a form, or requesting an appointment does not create a provider–patient relationship. A relationship begins only after an appointment is confirmed and a clinician evaluates you.
Not for emergencies
This site, our forms, chat, and text messages are not monitored for emergencies. If you are in crisis, call or text 988. For a medical emergency, call 911.
Appointment requests
Submitting a request does not guarantee an appointment. We review each request and may decline or recommend a different level of care based on clinical fit, licensure, or availability. Telehealth is available only to patients physically located in Florida at the time of the visit.
Text messaging (SMS)
By providing your mobile number through this site or our forms, you consent to receive appointment, scheduling, and account-related text messages from Let's Thrive Mental Health, which may be sent using automated technology. Consent is not a condition of receiving care. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time or HELP for assistance. We do not sell, rent, or share mobile numbers or SMS consent with third parties for marketing purposes.
Communications
You agree we may contact you by phone, email, or text about your inquiry, scheduling, and account. Email and standard text are not fully secure; please do not send sensitive health information through them. Use the patient portal for secure messaging.
Fees and cancellations
Self-pay fees, insurance billing, and our cancellation and no-show policy are provided before your first visit and in the patient portal. A card on file may be required to hold an appointment.
Accuracy
You agree the information you provide is accurate and that you are authorized to provide it, including when requesting care for a minor or another adult.
Intellectual property
Site content is owned by LETSTHRIVEMH CORPORATION and may not be copied without permission.
Limitation of liability
To the fullest extent permitted by law, we are not liable for damages arising from use of this site or reliance on its content.
Changes
We may update these terms at any time; the current version is always posted here.
