Three treatments · one evaluation decides
Which one, and why — told to you honestly.
All three work on depression that hasn't responded to standard medication. They differ in speed, precision, cost and coverage. We recommend based on your history — and we'll tell you when IV ketamine is the stronger option, and when it isn't.
$249 self-pay
TMS
Non-medication magnetic stimulation, 6 weeks of daily sessions. Insurance-covered. Planned — waitlist open.
Featured · fastest onset
IV Ketamine
Individualized dosing, adjusted in real time by an anesthesia-trained clinician. Relief often within hours to days; a 6-infusion series over 2–3 weeks.
Insurance + self-pay
Spravato® (esketamine)
FDA-approved nasal spray, often insurance-covered. Twice weekly for 4 weeks, then weekly. Fixed doses, monitored in-suite.

TMS
Non-medication magnetic stimulation, 6 weeks of daily sessions. Insurance-covered. Planned — waitlist open.
Precision you can't get from a fixed-dose spray.
Faster
Full IV bioavailability — effects have emerged within hours in controlled research. Spravato® typically takes days to weeks.
More precise
Dose is titrated to you, minute by minute, by a clinician trained in anesthesia — not chosen from two fixed strengths.
Safer in the right hands
Continuous monitoring and airway-trained oversight in a private suite. Same standards as an operating room, without the operating room.

Spravato® is still a good choice when insurance coverage matters most, when you prefer an FDA-labeled protocol, or when you'll benefit from the structured twice-weekly rhythm. We'll say so plainly.

Investment. Pricing is reviewed with you at your consultation and confirmed in writing before anything begins — one number, no surprises, financing available. Superbills provided for out-of-network reimbursement, and founding rates are locked for existing patients.
IV ketamine for depression is an off-label use of an FDA-approved anesthetic; ketamine itself is not FDA-approved to treat depression. Consultation required. Side effects are usually temporary and may include dizziness, dissociation, nausea and blood-pressure changes. A ride home is required after treatment.
Series, not single sessions
Priced as a course, because that's how it works.
Ketamine's effect builds across a series; one infusion tells us little. Every plan is a committed course with installments available, and the eligibility consult credits toward it.

Spravato® · the insurance-covered path
How Spravato® treatment works here.
FDA-approved esketamine, administered in-suite under the REMS program — prior authorization handled for you.
Who it's for
You may be a candidate if
-
Two or more antidepressants haven't brought remission
-
Depression with urgent suicidal thoughts needing rapid relief
-
Partial responses that keep fading
-
You want an insurance-covered next step before self-pay options
-
You can arrange a ride home after each visit
Not a fit: uncontrolled blood pressure, aneurysmal vascular disease, pregnancy, active substance misuse — screened at evaluation.
How a visit works
Blood pressure check → self-administered spray under supervision → about two hours of monitored rest in a private chair → discharge to your ride. Most people read, listen to music, or sleep.
Schedule
Weeks 1–4 · Induction Two per week
Weeks 5–8 Once per week
Weekly or every 2 weeks, individualized
Per Spravato® prescribing information; full safety information at spravato.com.
Safety, plainly
Spravato® is dispensed only through the REMS program at certified sites like ours. Each visit includes at least two hours of monitoring for sedation, dissociation, changes in breathing and blood pressure. You cannot drive until the next day after a restful night of sleep. Like ketamine, esketamine carries a risk of misuse; it is not taken home. Not for people with aneurysmal vascular disease, intracerebral hemorrhage history, or uncontrolled hypertension; discussed at your eligibility consult. Full prescribing information at spravato.com.
Cost & insurance
Spravato® is FDA-approved for treatment-resistant depression (alone or with an oral antidepressant) and is covered by many commercial plans and Medicare with prior authorization; criteria vary by plan. We verify and submit before you're scheduled. Self-pay quoted in writing at consult.
Ketamine for chronic pain
When pain keeps running the day.
Most people don't need therapy forever — they need the right kind, done deliberately, with a way to tell whether it's working. Sessions here use evidence-based approaches matched to your goals, and progress is tracked with the same measures your prescriber sees.
CRPS
One of the most studied pain indications; short-term analgesic benefit in randomized trial
Neuropathic pain
Benefit shown in some populations; durability and effect size vary.
Refractory chronic
An option after multidisciplinary treatment has not provided enough relief.
Off-label use: ketamine is FDA-approved as an anesthetic, not as a chronic-pain medication. Protocols vary substantially by diagnosis; medical screening and monitoring are essential.

Asked often. Answered clearly.
Psychiatric visits are billed to participating insurance plans or self-pay. Wellness programs use transparent flat self-pay pricing reviewed with you before enrollment — no surprise bills. Optional membership is $199 or $399 per month, published right on this page.
Dr. Borrego is a doctorate-prepared psychiatric nurse practitioner (DNP, PMHNP-BC) and certified registered nurse anesthetist (CRNA) — licensed in Florida to evaluate, diagnose, and prescribe for psychiatric conditions. The dual training in psychiatry and anesthesia is exactly what allows IV ketamine and Spravato® care to be delivered in-house by the same clinician who manages your psychiatric plan.
Start with a psychiatric evaluation if mental health is the main concern. Choose the $249 Precision Assessment if the picture is broader or unclear, or use the 2-minute fit assessment for a quick directional starting point.
Many psychiatry, therapy, and wellness services can be delivered by telehealth to eligible patients in Florida. IV ketamine, Spravato®, TMS, and IV lounge services require in-person care in Coral Gables.
