Does insurance cover Spravato® in Florida?

By Jay Borrego, DNP, PMHNP-BC, CRNA · Written and clinically reviewed by the author · Published September 2026 · Last clinical review: September 3, 2026 · Coral Gables, FL

Usually, yes — and that's the short answer most clinics make you call to get. Spravato® (esketamine) is FDA-approved for treatment-resistant depression and for depressive symptoms with acute suicidal ideation, so commercial plans and Medicare generally cover it when criteria are met.

What "criteria" means

Coverage criteria vary by plan; most want documentation of two adequate antidepressant trials and treatment at a REMS-certified site, and some ask for more than the FDA indication requires. That paperwork is the prior authorization — and a practice that handles it for you, before your first visit, removes the single biggest delay in starting.

What you actually pay

With coverage, the typical patient owes a specialist copay per treatment visit; manufacturer savings programs can reduce commercial copays further. Without coverage, self-pay is quoted in writing at your consult. Either way, the number should never be a surprise on a bill.

The Florida wrinkle

Telehealth can handle the evaluation and follow-ups, but Spravato® itself is always administered in office with about two hours of observation — so choose a site you can actually get to twice weekly during induction. Ours is in Coral Gables, with the schedule built around working patients.

Start with a Spravato® eligibility check — we verify your benefits and review your expected cost before the visit.
